From Collapse to Comeback: A Successful VV ECMO Journey in Severe ARDS

Author Details
Name: Dr. Vijaya Lanje
Designation: Clinical
Perfusionist & ECMO Coordinator
Institution: KIMS Kingsway Hospital, Nagpur

Introduction

Severe ARDS remains one of the most challenging conditions in critical care. When conventional ventilation fails, ECMO becomes a lifesaving bridge. This case highlights the importance of timely decision-making and multidisciplinary teamwork in achieving a successful outcome.

Patient Profile

– Age/Gender: 62-year-old Male
– Diagnosis: Severe ARDS secondary to viral pneumonia
– Indication for ECMO: Refractory hypoxemia despite optimal ventilatory support and Prone trials…

Clinical Journey

The patient presented with rapidly worsening hypoxia and respiratory distress. Despite lung-protective ventilation, prone positioning, and maximal medical management, oxygenation remained critically low.

After multidisciplinary discussion, VV ECMO was initiated as a rescue therapy. Cannulation was performed via femoro-jugular approach, and adequate flows were achieved.

Challenges Faced

– Persistent hypoxemia in initial hours
– Hemodynamic instability during cannulation
– Anticoagulation balancing due to fluctuating ACT levels
– Coordination between ICU, perfusion, and nursing teams

ECMO Management Highlights

– Early initiation of lung-protective ventilation strategy
– Careful anticoagulation monitoring and titration
– Regular assessment of oxygen delivery and circuit performance
– Strict infection control and fluid balance management

Outcome

Gradual improvement in lung compliance and gas exchange was observed. After 9 days of ECMO support, the patient was successfully weaned off ECMO.
The patient was later extubated and discharged in stable condition, highlighting the effectiveness of Right ECMO intervention at right time .

Key Learning Points

– Early ECMO consideration improves outcomes in severe ARDS
– Team coordination is crucial for successful ECMO management
– Continuous monitoring and timely troubleshooting prevent complications
– Individualized patient care is key to recovery

Take-Home Message

“ECMO is not just a machine—it is a team-driven lifesaving strategy when applied at the right time.”
